Saturated fat, ApoB and the 10%-of-energy ceiling

The saturated-fat debate never ends, but the actionable version is simpler than the argument. Here is why the marker to watch is ApoB, why the guideline ceiling is expressed as a percentage of energy, and how Misi tracks both.

Saturated fat is the nutrient the internet cannot agree on. Rather than relitigate the entire debate, it is more useful to anchor on two things that are not seriously contested: saturated fat raises LDL and ApoB in most people, and ApoB is the better predictor of cardiovascular risk. From there the practical guidance falls out cleanly.

Why ApoB, not just LDL cholesterol

Every atherogenic lipoprotein — LDL, VLDL, Lp(a) — carries exactly one ApoB molecule, so ApoB counts the total number of artery-damaging particles rather than the cholesterol they happen to carry. Sniderman and colleagues (2019, JAMA Cardiology) showed ApoB outperforms LDL-C when the two disagree, which is common in people with insulin resistance or high triglycerides. If you can measure only one lipid marker to guide fat intake, ApoB is the one.

LDL-C counts the cholesterol; ApoB counts the particles. When they disagree, the particle count wins.

Why the guideline is a percentage, not a gram figure

The American Heart Association's 2021 dietary guidance (Lichtenstein et al., Circulation) frames the ceiling as saturated fat below about 10% of total energy — with 7% often cited as heart-optimal — because a gram target makes no sense across a 1,800 and a 3,200 kcal diet. Mensink's WHO-commissioned meta-analysis (2016) confirmed that replacing saturated fat with polyunsaturated fat lowers LDL and cardiovascular events; swapping it for refined carbohydrate does not. The lever is replacement, not just restriction.

You do not need to fear a slice of cheese. You do need to know roughly what share of your energy is coming from saturated fat over a week, and — if your ApoB is up — to replace some of it with olive oil, nuts and oily fish rather than with 'low-fat' refined carbohydrate.
