Below 1.0 indicates optimal insulin sensitivity. 1.0–1.9 is the normal range for healthy adults. 2.0–2.9 suggests early insulin resistance, and 3.0 or above indicates significant insulin resistance that warrants clinical follow-up.

Two values from the same fasted blood draw: fasting glucose and fasting insulin. Fasting insulin is not on most standard panels — you may need to request it, but it is inexpensive and it catches insulin resistance up to a decade before glucose or HbA1c drift out of range.

The TyG index (ln[triglycerides × glucose ÷ 2]) is a validated insulin-resistance proxy that needs no insulin measurement. A value of 8.8 or higher flags risk. Because triglycerides are on every standard lipid panel, TyG can flag insulin resistance from labs you already have.
