Keto without wrecking your lipids: who it suits and who should watch their ApoB
A ketogenic diet works well for some people and backfires for others — and one of the most important variables is what it does to your cholesterol. Here is how to run keto while keeping an eye on the marker that matters.

They also produce a highly variable lipid response, and for a subset of people that response is a problem worth taking seriously rather than dismissing.
What keto actually is, numerically
Nutritional ketosis generally requires carbohydrate low enough — often under 20 to 50 g/day — that the body shifts to producing ketones for fuel. In macro terms that typically lands around 50 to 70% of energy from fat, 25 to 35% from protein, and under 10% (often under 5%) from carbohydrate. The metabolic benefits for glucose and insulin are well documented; the fat composition is where the caution lives.
The lipid wildcard
For many people, LDL and ApoB stay stable or even improve on keto, especially alongside weight loss. But a distinct group — often lean, metabolically healthy individuals, described in the 'lean mass hyper-responder' literature (Norwitz et al., 2022) — see LDL and ApoB rise sharply, sometimes dramatically, when fat intake is high and saturated fat dominates it. Since ApoB is the better predictor of cardiovascular risk, a large rise is not something to wave away as 'just keto'.
Keto's metabolic wins are real. So is the ApoB rise some people get — and the only way to know which camp you're in is to test.
How to run it more safely
If you do keto, favour monounsaturated and polyunsaturated fats — olive oil, avocado, nuts, oily fish — over a foundation of butter, cheese and fatty red meat, and re-test ApoB within a few months of starting. If ApoB climbs meaningfully, that is real information: shift the fat sources, or reconsider whether a less extreme low-carb approach gets you the glucose benefit without the lipid cost.
