Does fasted cardio actually burn more fat? What a controlled trial found
"Should I do cardio before breakfast to burn more fat?" is a long-running gym myth. A four-week randomized trial testing fasted versus fed aerobic exercise, on an identical calorie-deficit diet, settles it.

The idea that fasted-state cardio — typically a session done first thing in the morning, before eating — burns more body fat than the exact same workout done after a meal has been gym folklore for decades. The logic sounds reasonable: lower blood sugar and glycogen should force the body to rely more on fat for fuel during the session itself. A 2014 randomized controlled trial in the Journal of the International Society of Sports Nutrition tested whether that shifted substrate use during a single workout actually changes body composition over time.
The experiment
Bret Contreras’s and Brad Schoenfeld’s team randomized 20 healthy young women into two groups: a fasted group that trained after an overnight fast, and a fed group that ate a meal-replacement shake before training. Both groups did one hour of steady-state aerobic exercise, three days a week, for four weeks — with the training volume equated between groups. Crucially, both groups followed the same customized, monitored hypocaloric (calorie-deficit) diet throughout, with dietary adherence tracked by a research assistant.
What actually happened to body composition
Both groups lost a significant amount of weight and fat mass over the four weeks — the calorie deficit did its job in both cases. But there was no significant difference between the fasted and fed groups in any body-composition outcome measured: not fat mass lost, not fat-free mass retained, nothing that would suggest fasted training carried any real-world fat-loss advantage over the same session done fed.
Body composition changes associated with aerobic exercise in conjunction with a hypocaloric diet are similar regardless whether or not an individual is fasted prior to training.
Why the substrate-use theory doesn’t translate to results
It is true that a fasted body draws relatively more of its fuel from fat during a given session — that part of the theory isn’t wrong. What the theory misses is that fat loss over weeks and months is governed by total energy balance across the whole day, not by which fuel source was slightly favored during one hour of cardio. A fed session that burns relatively more glycogen simply shifts your body’s fuel-partitioning math elsewhere in the following 23 hours — the net effect on stored body fat, over a real calorie deficit sustained over time, washes out to the same result, exactly as this trial found.
The practical takeaway
Whether you train fasted or fed is a legitimate personal-preference and comfort decision — some people feel lighter and perform better fasted, others feel weak or lightheaded without eating first — but it is not a fat-loss lever worth stressing over or restructuring your schedule around. The variables that actually determine fat loss are your total calorie balance across the day, your protein intake, and your training consistency, not the timing of your first meal relative to a cardio session.
