Not all fat is metabolically equal. Visceral fat — the fat packed around abdominal organs rather than under the skin — drains directly into the portal vein and drives inflammation and insulin resistance in a way subcutaneous fat does not. A 2026 Circulation paper gives the longest MRI-based look yet at what happens to visceral fat, and to the people carrying it, a full decade after a structured diet intervention.
What the follow-up did
The researchers followed participants from two earlier randomised dietary trials (CENTRAL and DIRECT-PLUS), re-scanning visceral fat by MRI at both 5 and 10 years after the original intervention ended, and linking those long-term visceral fat trajectories to cardiometabolic outcomes over the same period.
Why visceral fat specifically predicted outcomes
Participants who achieved and sustained a meaningful reduction in visceral fat during the original trial period showed better cardiometabolic health markers a decade later — independent of how much total body weight had fluctuated in between. That decoupling is the key finding: it is durable visceral fat reduction, not total scale weight, that tracked with long-term cardiometabolic health.
Practical takeaway
